How to Refer Someone to Able Options

Referrals to Able Options can come from more places than people expect — and you don't need to be an NDIS expert to get one started.
Who can refer someone to us:
The participant themselves (self-referral — you're always in control of your own choices)
A family member or carer, with the participant's consent
A Support Coordinator or Local Area Coordinator (LAC)
A Plan Manager
A GP, hospital, or allied health professional — for example, during a hospital discharge or a major life transition
When it usually makes sense to refer:
Right after an NDIS plan is approved
When starting new supports, or changing providers
When a participant's goals or needs have changed
During a hospital discharge or another significant transition
What to have ready before you reach out:
The participant's NDIS goals
Which supports are funded in their plan
Any specific needs — mobility, communication, cultural, or otherwise
Contact details for the participant and whoever's coordinating on their behalf
The referral process:
Get in touch — call, email, or fill out our online referral form
We'll have a short conversation to understand the participant's needs and goals
We'll confirm what's funded in their plan and how our supports can help
We'll arrange a time to meet the participant and start setting up their supports
Ready to make a referral? Call us on 0402 544 401 or 0410 030 565, or email admin@ableoptions.com.au and we'll take it from there.
